Thinking about Changing Your Name?
Legally changing your name in the State of Michigan involves submitting a form and getting permission from the courts. Remember that you must also change your name with the Federal Government!
To change your name with the Social Security Administration, first complete the “Application For A Social Security Card.” This form is called an “SS-5.” You must also show proof that you legally changed your name (usually a document submitted to and processed by the courts). You must also prove your identity (with documents like a passport or drivers license), and prove that you are a US Citizen or an approved immigrant.
All your documents must be originals, or be certified by the agency that issued them. The Social Security Office will not accept copies or uncertified copies. Take your application and documents to a Social Security office for processing. Your new name will appear on your new social security card, but in most cases you will retain your original Social Security number. If you mail your documents in, they will be returned by mail once the file is complete and processed.
